Re:                  HPD SECTION 8 VOUCHER PROGRAM – WORKSHOP

HPD has Section 8 vouchers available for those who qualify!

HPD and Management will be in the Community Room of Building 2 (59-15 47th Avenue, Woodside, NY 11377) on Thursday July 18th from 3pm to 7pm and Saturday July 20th from 10am to 3pm assisting residents who qualify for Section 8 receive an application for the subsidy.

Step 1: Only the head of household must report to the Management Office BEFORE the two scheduled events with their social security number to be added to the qualified Section 8 list. A list of qualified shareholders must be given to HPD prior to the events in order to generate the applications the day of the workshop.

Step 2: If you do qualify for Section 8 after completing step 1 (Management will verify if you qualify) you will attend the workshop events and bring the attached documents required by HPD. They will need each household member’s information for this step. (Documents required by HPD are attached to this memo)

DOCUMENTS NEEDED TO APPLY FOR SECTION 8

(Please try and make copies. HPD will be taking the requested documents with them the day of the event)

1. Proof of social security number for all residents: (If a household member has never been issued a Social Security number or if you cannot provide a social security card you can provide these)

IRS FORM 1099, public assistance letters, or court records stating name and Social Security number
Government Issued ID or letter stating name and Social Security number
Social Security benefits letter Medicare or Medicaid card or letter

2. Proof of Income: Most recent and consecutive paystubs (attach 2 if paid monthly, 3 if paid biweekly, 6 if paid weekly)
Most recent Social Security award letter
Most recent pension award letter
Most recent public assistance budget letter
Written child support agreement

3. Proof of Assets:
Copies of most recent bank statement for each account
Copies of bank passbooks
Broker’s quarterly statement showing value of stocks

4. Proof of Date of Birth:
Birth Certificates, Passports, Driver’s license, other government issued identification
Baptismal certificate (you may submit this only if you cannot provide other documentation)

5. Proof of identity – Attach copy of photo identification for each household member 18 years of age or older.
Driver’s license, Passport or Other government issued identification.
If a household member does not have valid photo identification he or she must submit a written statement explaining why he or she cannot provide it or why it is out date. The house hold member must sign and date the statement

6. Proof of Citizenship and Eligible Immigration Status for Non-Citizens:
U.S. Birth certificate, US Passport, Certificate of Citizenship or Certificate of U.S. Naturalization
Attach proof of immigration status for each house hold member that declares themselves a legal resident such as:
Copy of valid permanent resident card (photocopy front and back)
USCIS/BCIS receipt that shows that individual entitlement has been verified (not that it is pending)
Copy of one of the following forms: I-1551, I-94, I-688, I-688B
